The DMV's registration form which is used for any plate. Simply fill out that form and send it to me with a check made out to DMV 
for the amount of the registration + $30 for the DMV* specialty plate fee.

*VERY IMPORTANT: Due to the fact that no plates will be produced until the first 400 are sold, the DMV will NOT be accepting these forms. So mail your form and check to:

along with a note containing your e-mail address, so I can notify you occasionally of how the process is going.

Your check(s) will not be cashed until all of the first 400 are sold.

I will notify you (if I have your e-mail address) when I will be sending all of the forms and checks to DMV.
